The Preliminary Information Form is one step in your relationship with South America Mission (SAM). It is not considered an application, and does not obligate you to become a member of SAM. || MORE
On the 5th of each month (or shortly thereafter) your gift will be electronically transferred from your account. We will send you a receipt twice a year for your records.
